Purpose of Conditions For Learning Survey

The Sioux City Community School District schools are getting ready to administer an annual school climate and culture survey called Conditions for Learning to all students in grades 3 through 12. This survey administration is required by the state’s plan to meet a federal law called the Every Student Succeeds Act. Each school will administer a survey between April 3-12, 2024.

All Iowa public school districts will use the Conditions for Learning Survey to improve safety, student engagement, and the overall learning environment so that all Iowa students have optimum conditions for learning. Our work to improve safety, student engagement, and the overall learning environment will have a lasting impact on the future.

We will ask your child to simply fill out a confidential survey.  A high rate of participation by students in completing the Conditions for Learning Survey is important for obtaining the most complete school climate data for our school.  Support from you in communicating to your students that this is their chance to be heard and to be part of the effort to improve conditions for learning at their school is critical.

Survey Information Packet & Written Refusal for Student

An information packet has been put together by the Department of Education that contains:

  • An information summary about the student surveys

  • A link to the survey questions

  • A written refusal form if you do not want your child/children to participate in completing the Survey

    • Please note, the written refusal form must be printed and returned to your child's school by April 2, 2024. If you do not have access to a printer, please call your child's school for further assistance.

Parent/Guardian Participation

The Department of Education has also asked that parents complete a short survey to broaden the scope of information garnered through this survey process.

Your participation in the survey means your school will have valuable feedback for identifying areas of strengths and areas for improvement. Follow the instructions below to complete the survey. It should take less than 20 minutes. You may submit one response for each child.

Parent/Guardian Survey Directions

  1. Select Your School

    1. On the main landing page, you will see two drop-down lists.

    2. If your student is in grades Preschool-5, select from the first list.

    3. If your student is in grades 6-12, select from the second list.

    4. When you have found the school at which your student attends, click the “Take This Survey” button to begin.

    5. On the next page, confirm that the district and school names are correct.

    6. If they are not correct, go back to select the correct school.

  2. Complete the Survey

    1. Once you have confirmed the district and school name are correct, read the instructions and begin answering the questions. All questions are optional, and you may skip any you do not want to answer. When you are done answering questions, be sure you click the blue “Submit” button at the end to submit your response. If you exit the window before doing this, your responses will not be saved and you will need to begin again.

  3. Optional - Submit Another Response

    1. If you would like to respond for another child after submitting your response, click on the “Take Another Survey” button. You will be directed back to the school selection page to start a new survey.
